Some school districts levy an income tax. If you lived in (resided) or were domiciled in one of these districts for all or part of the year, you are required to file a school district income tax return, Ohio form SD 100. See the School District Income Tax website for more information.

  • Step 1 - Go to School District Income Tax page.
  • Step 2 - Search by address, Zip Code, or latitude/longitude.
  • Step 3 - The Ohio public school district name and number will appear on the screen with the applicable tax rate if the school district has an income tax in effect.
  • Step 4 - Enter the Ohio public school district number in the space provided on your Ohio form IT 1040 or SD 100.
  • Step 5 - If the income tax rate in Step 4 is anything other than 0.00%, pay the tax using the School District Income Tax Form, available as a PDF or PDF Fill-in. Instructions are available.

Where do I mail my forms?

Always sign your return before mailing. If you are enclosing a payment with your return, be sure to enclose Ohio form IT 40P/IT 40XP (or SD 40P/SD 40XP) with your check or money order. This will ensure proper crediting of your payment. For a full list of mailing addresses, see this page.

Individual Income Taxpayer Assistance

General assistance by telephone: 800-282-1780 (8:00 a.m. - 5:00 p.m. Monday through Friday). Extended telephone hours until 7 p.m. Monday through Friday will be available from April 4 through April 18. Walk-in assistance is available at the Taxpayer Service Center (4485 Northland Ridge Blvd., 1st Floor, Columbus, OH 43229) from 8 a.m to 5 p.m. Monday through Friday.

Refunds

If you submitted your state income tax return by mail, it will take some time to process. Check on the status of your refund online here; by calling the automated refund hotline at (800) 282-1784; or speaking to a live representative at 800-282-1780. You must enter your social security number, your date of birth, the type of tax and whether it is an amended return.
